When you contact me, I will take note of your email and telephone contact number. This is to both send information on the assessment appointment and in case of any cancellations. This is stored securely under password protection (email/smartphone).
At assessment, I will collect your address, contact number, date of birth and GP details. These form part of our record while counselling is ongoing and for the purposes of communication about sessions. The DOB and GP details are in case of need to breach confidentiality (See What I Offer/ Contract). Counselling can not be offered without these details, which are for your own safety. The details are securely stored and not shared with anyone else, or used for purposes such as marketing.
I keep brief written (paper) notes of our sessions; these are for supervision purposes, are stored securely and are not identifiable by name. Notes are kept, under BACP guidelines, for up to 6 years after counselling has ceased.
I keep accounts which identifiy you only by initial. This is to keep track of payments and is not shared anywhere else.
In the case of emergency which renders me unable to work, you may be contacted by my supervisor. Your basic contact information will be shared at this point, by secure email, or phone, but only for the purpose of communicating about counselling.
